  • Name of the Person on the Certificate
  • Type of Sacramental Record Request (Ex. Baptism, Confirmation)
  • Birthday of Person on the Certificate (Not including Sacrament of Matrimony)
  • Full Name of the Father and Mother's MAIDEN name
  • Approximate Date/Year of Sacrament
  • Best way to contact Requester (ex. email, telephone number)

    Looking for a Sacramental Record from 'Resurrection Parish' (1997 - 2014)?

    Please contact the Parish Office today.

    Resurrection Parish was formed in 1997, consisting of five churches, St. Peter's, St. Boniface, St. Bridget, St. Michael and St. Mary. In February 2014, the parish cluster was suppressed. St. Michael and St. Mary became St. Michael's Parish and St. Mary's Parish respectively, once again.

    All Sacramental records for the following closed Churches (Pre & Post-Resurrection Parish, Before 1997):

    • St. Bridget's Church (Closed, 2015)
    • St. Peter's Church (Closed, 2009)
    • St. Boniface Church (Closed, 2006)
    • St. Lucy's Church (Closed, 1986)

    are kept at Walsh Library - Seton Hall University.
